Liability Waiver, Release of Liability, and Assumption of Risk

In consideration of being permitted to participate in training, classes, events, and related activities at MKJ Academy, hereinafter referred to as "THE GYM", I, the undersigned, hereby agree as follows:
1. Assumption of Risk
I understand and acknowledge that martial arts and combat sports training, including but not limited to Brazilian Jiu-Jitsu, MMA, Muay Thai, Wrestling, sparring, grappling, striking, conditioning, and any related activities at THE GYM, involve inherent and significant risks of serious injury, permanent disability, disfigurement, and even death. These risks include, but are not limited to: strains, sprains, fractures, concussions, traumatic brain injuries, joint injuries, spinal injuries, transmission of skin infections (such as MRSA, ringworm, impetigo), dehydration, exhaustion, and other foreseeable and unforeseeable risks.
I voluntarily assume all such risks, whether known or unknown, and agree that my participation in activities at THE GYM is entirely at my own risk. I understand that no amount of safety equipment or supervision can eliminate these risks.
2. Release of Liability
I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue THE GYM, its owners, officers, directors, members, employees, independent contractors, agents, volunteers, affiliates, successors, and assigns (collectively referred to as the "Released Parties") from any and all liability, claims, demands, actions, or causes of action whatsoever, whether for ordinary negligence or otherwise, arising out of or related to my participation in activities at THE GYM, including but not limited to any injury, loss, or damage to person or property.
This release includes claims arising from the negligence of the Released Parties, premises liability, equipment failure, inadequate instruction or supervision, and any other acts or omissions.
3. Medical Acknowledgment and Authorization
I certify that I am in good physical condition, have no medical conditions that would prevent me from safely participating, and have consulted a physician if I have any doubts. I agree to immediately notify THE GYM of any changes in my health status. In the event of an emergency, I authorize THE GYM staff to seek medical treatment on my behalf, and I agree to be responsible for all related costs.
4. Rules, Conduct, and Safety
I agree to abide by all rules, policies, and instructions of THE GYM and its instructors at all times. I understand that failure to do so may result in my removal from the premises without refund and may increase my risk of injury.
5. Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, losses, or expenses (including reasonable attorney fees) arising from my actions or negligence while at THE GYM or participating in its activities.
6. Additional Provisions
I have carefully read this entire agreement, fully understand its terms, and sign it voluntarily. I am aware that I am waiving substantial legal rights.
This agreement shall be governed by the laws of the State of Oklahoma.
If any provision of this agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
This document contains the entire agreement between the parties and supersedes any prior understandings.
